This is the Mantis Head from Joss Whedon's Buffy: The Vampire Slayer. This is the Creature from the first season of Buffy in the episode entitled "Teacher's Pet." The teacher, Natalie French, turns into the giant praying mantis and wants to impregnate the male students. She held Xander captive in her basement cell. Buffy kills her with bug spray and plays bat sonar to affect the nervous system. To end all, Buffy kills her with a machete. The fiberglass Mantis Head is just over 20"(51cm) wide with gold and green accents, there is light wear from production use but it's still in fairly good condition.
